The Dr. Rafael Angel Calderon Hospital belongs to the public health system of the Costa Rican Social Security Fund (CCSS) and is classified as a class A hospital, which places it among the highest level hospitals in the country. It was originally inaugurated in 1943 as the Polyclinic, evolving in 1972 to its current name in honour of Dr. Rafael Ángel Calderón Guardia, who was the reformist president and founder of the CCSS.

It is one of the leading medical centres in the country, offering 24-hour care, emergency, hospitalisation, surgery and specialised care. It has also been recognised for its environmental management and commitment to sustainability. In 2022 it received awards in waste and sustainable procurement categories from the Global Network of Green and Healthy Hospitals. The hospital has a Haematology-Oncology Department that integrates the services of Medical Oncology, Surgical Oncology and Haematology. Since 2024 it has had a Radiotherapy service

The San Juan de Dios Hospital is a public hospital belonging to the Costa Rican Social Security Fund (CCSS). Founded in 1845 and in operation since 1855, it is the oldest hospital in the country and one of the most important in the country.

It is a comprehensive referral centre with all medical-surgical specialties, including oncology, surgery, internal medicine, emergency, intensive care, diagnostic imaging and multi-purpose care. It offers medical and surgical oncology care, chemotherapy treatments, radiotherapy and multidisciplinary care. It was one of the first hospitals in the country to incorporate oncology and radiotherapy services in the 1940s. It functions as a teaching hospital with continuing medical education and specialised care for highly complex patients.

The San Vicente de Paúl Hospital is a public hospital belonging to the Costa Rican Social Security Fund (CCSS). Founded in 1872, the hospital was moved in 2010 to new modern facilities comprising six buildings covering approximately 11 hectares. It offers outpatient, emergency, surgical, inpatient, diagnostic imaging, pharmacy and laboratory services.

It is part of the regional hospitals of the CCSS, which allows it to attend to sub-specialties and cases of greater complexity at the regional level. It has around 42 consulting rooms, 9 operating theatres, 10 emergency rooms, 5 delivery rooms, 3 dental offices, and multiple diagnostic rooms.

It offers emergency care, hospitalisation, internal medicine, general surgery, gynaecology and obstetrics, paediatrics, dentistry, clinical laboratory, pharmacy, X-ray, imaging and other medical and surgical specialties.

Hospital Mexico, administered by the Caja Costarricense del Seguro Social (CCSS), is one of the Class A general hospitals in San José, with approximately 633 beds and nationally referenced emergency and trauma services.

It has a Haematology-Oncology Department that integrates medical, surgical, radiotherapy, breast and digestive surgery, sarcoma and other specialised oncology services. It operates through specialised units by tumour type (breast, digestive, sarcoma, hepatobiliary, endocrine), which streamlines diagnosis and treatment during the same cycle of care.

From November 2023, the hospital offers robotic radiosurgery services (CyberKnife) for various types of tumours (brain, lung, spine, pancreas, liver, prostate), with precise, outpatient treatment.
